Please read this entire document before you sign it. I hereby acknowledge that I am a volunteer at Bridge House and not an employee. I understand and acknowledge that my participation in any and all work activity at or for Bridge House is completely voluntary on my part and is performed without the expectation of payment or remuneration. Further, I understand and acknowledge that I am not eligible for benefits of any kind from Bridge House as a volunteer. This includes health insurance, dental insurance, unemployment benefits and workers’ compensation benefits. As a volunteer, I agree to maintain my own health insurance during my time as a volunteer for Bridge House. I recognize and accept that volunteering has risks and potential unforeseen dangers. I have read the detailed explanation of the volunteer duties I am accepting and understand the minimum requirements. I understand that I have the right to review each activity prior to my participation and choose to participate of my own free will. I have read and understand Bridge House’s mission statement and best practice procedures. I pledge to act and perform within those expectations. I hereby waive any and all claims for damage or loss to my person or property, which may be caused by Bridge House, including its directors, officers, employees, agents, and/or affiliated organizations. Further, I hereby release all the aforementioned parties from liability, claims, demands, costs, charges and expenses incident to any possible damage or personal injuries sustained by me while working as a volunteer for Bridge House. I voluntarily assume the risk of all dangerous conditions associated with my work as a volunteer for Bridge House and waive any and all specific notice of the existence of such conditions. I promise not to assert against Bridge House including its directors, officers, employees, agents, and/or affiliated organizations, their heirs or legal representatives, in any court of law, any claim or claims that I now have or may have in the future, known or unknown, based on any injury to my person or property as a result of my participation in volunteer work for Bridge House. I acknowledge that, since I am a volunteer and not an employee, in the event of an injury during the course of my volunteer work for Bride House I will not be entitled to receive any Workers’ Compensation benefits, and in the event that my volunteer services are no longer needed by the Club, I will not be entitled to receive any Unemployment Compensation benefits. By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made my me on this application may result in my immediate dismissal.
